A bilobed ap can be designed if donor site closure of the
primary ap is not going to be possible. A second transverse-
ly orientated ap can be raised and transposed 90 degrees
to close the vertically orientated primary donor site.

perforators pass through the septum lying between the tensor
fascia lata (TFL) and gluteus medius muscle located in the
anteromedial portion of the TFL muscle. The musculocutaneous
perforators pierce the TFL muscle. Both perforators can be used as
the vascular pedicle of the TFL-PBIF.
